What we can and cannot accept

Stated plainly so you don't spend a call finding out. If a client falls outside this, tell us anyway — we would rather refer you onward than waste your time.

We routinely accept

  • Alzheimer's and other dementias, early through late stage
  • Exit-seeking and wandering behaviors
  • Sundowning and evening agitation
  • Full assistance with all activities of daily living
  • Incontinence care, including supplies
  • Medication administration and management
  • Hospice on site, in the resident's own room
  • Post-hospital and post-rehab transitions
  • Respite and short-term stays

Outside our scope

  • Ventilator or tracheostomy care
  • Behaviors presenting a danger to other residents
  • Active substance withdrawal management
  • Skilled nursing level of care
  • Bariatric equipment requirements
  • Court-ordered or secure psychiatric placement

Edge cases are a conversation, not a form. Paige will review clinicals directly and give you a yes or no the same day.

How a referral works

01

Call or send clinicals

Reach Paige directly. Send a face sheet, medication list, and recent history — whatever you have. No portal, no intake queue.

02

Same-day clinical review

A nurse practitioner reviews the file, not an admissions coordinator. You get a clear yes, no, or a specific question — same day in nearly all cases.

03

Family tour, often within 24 hours

We accommodate urgent discharge timelines. Families tour privately, and we are candid with them about fit.

04

Move-in and assessment

Care plan built with the family before arrival. Medication reconciliation on day one. We coordinate directly with the resident's physician and any incoming hospice or home health team.

05

You hear back

We close the loop with the referring professional after placement. You should know how your client settled in.

Why professionals use us

01

Clinical judgment at the top

Both homes are owned by a Doctor of Nursing Practice and board-certified FNP. Care plans, change-of-condition reviews, and medication oversight run through a clinician, not a manager.

02

Ten residents, not sixty

Your client is known by name within a day. Changes get noticed the shift they start, not at the next care conference.

03

Pricing that does not surprise families

All-inclusive from $6,500. Care, meals, incontinence supplies, and household needs are included — no level-of-care surcharges as the disease progresses.

04

No gatekeepers

You reach ownership directly. Decisions happen on your discharge timeline, not on a corporate approval cycle.

A direct line, not an intake queue

For an urgent discharge or a clinical question about fit, call or email Paige directly. If she is with residents you will hear back the same day.
